Southern Heart Group has been caring for the heart health of Northeast Florida residents for over 30 years and we are pleased to have you join us as a patient. We look forward to seeing you at your first appointment as we strive to make your visit both pleasant and worry free.

Please try to complete each form as accurately and thoroughly as possible and don’t hesitate to contact us if you have any questions. For your first appointment, please bring the completed paperwork with you along with all of your current medications, picture identification, and your insurance card(s).

Patient Education
Heart disease can take the form of any number of conditions that affect the heart and cardiovascular system. This section provides easy-to-understand descriptions of the most common heart disease conditions, including heart attack, congestive heart failure, and hypertension (high blood pressure), among others. You’ll also find some of the most common tests that your doctor may use to help diagnose symptoms you may be having, or to rule out the presence of heart disease. Information on the most common treatment approaches to the management of heart disease is also provided. Remember, the more you know about your condition and treatment options, the better off you will be.
